Exercise-Induced Headaches: What's Normal, What's Not, and How to Prevent Them
If you've noticed a dull, pounding ache creep in partway through a hard set, don’t panic. You're not injured, and you’re not necessarily overtraining. You may be dealing with an exercise-induced headache, a real and reasonably well-studied phenomenon. The good news: for most people, it's manageable, predictable, and preventable once you understand what's actually happening.

Exercise-induced headaches usually come from a temporary spike in blood pressure and cranial pressure during hard effort — uncomfortable, but rarely dangerous.
The most common triggers are dehydration, poor warm-ups, heat, and going out too hard too fast — all preventable.

What's Actually Happening in Your Head
Exercise-induced (or "exertional") headaches happen when intense physical effort causes a rapid rise in blood pressure, which in turn increases pressure inside the skull. Cleveland Clinic explains that during exertion, your blood vessels dilate to push more blood through the body, and that expansion is what generates the throbbing sensation.
Some researchers also point to a more specific culprit: in a subset of people, valves in the jugular vein don't fully close during straining, allowing blood to flow backward and briefly congest the veins draining the brain. Explosive, unpredictable effort is the other major trigger. A hard, sudden strain — think a maximal Leg Press attempt — spikes blood pressure faster than your vascular system can adjust. While this probably sounds like you should drop your weight and run, it actually turns into one of the advantages of ARX adaptive resistance training. Because you can control the rate of force application, you can make sure you are adjusting your technique to avoid causing yourself this issue.

“Don’t jazz the clutch,” as I’m fond of saying to people I see “spiking” their ARX set. It’s the same as driving a manual car. You want to ease onto that pressure instead of “jamming” the weight. You’ll end up with better numbers, and you’ll avoid this headache issue.
Prevention: The Boring Stuff That Actually Works
A US Pharmacist clinical review on exercise headaches lays out prevention steps that are refreshingly unglamorous: a proper warm-up before training, consistent hydration, avoiding training in extreme heat, and — for people with a known pattern — taking an anti-inflammatory 30 to 60 minutes before a workout that reliably triggers one.
None of this requires new equipment or a dramatic overhaul of your training. It requires consistency: five to ten minutes of gradually escalating movement before you touch a heavy load (we can adjust your set to include more mid-level lifts before we attempt the lift that’s causing the spike), water in your system well before your session starts (not chugged five minutes prior), and a realistic read on the day's conditions if you're training outdoors in the heat and humidity.
When to Push Through, and When to Stop
Most exertional headaches are what's called "benign" — uncomfortable but not dangerous, typically resolving within minutes to a couple of hours after you stop exerting. That said, Mayo Clinic is clear that certain symptoms warrant an immediate stop and a call to your doctor: a headache that's the worst of your life, sudden onset with vision changes, confusion, neck stiffness, or a headache after a head injury. These aren't the profile of a typical exertional headache — they're signs of something that needs medical evaluation, not modified training.
